We use monthly billing. Payment is due by the 1st of each month via credit card on file (3% fee) or cash/check. A one-month deposit is required upon signup and will be applied to the final month in May. A 30-day written notice is required to cancel and must be submitted via email to admin@netatennisacademy.com; if notice is not submitted, the deposit will be forfeited. Make-ups are not guaranteed and, if accommodated, must be completed within the same month. No refunds or credits will be provided for missed classes.

I will not hold Park Avenue Tennis Center LLC and NETA or any instructor liable for any injury or accident that occurs while my child is participating in this program.
In case of an injury, I grant Park Avenue Tennis Center LLC and NETA Staff permission to administer standard first aid treatment on site or to transport my child to the nearest medical facility equipped to handle the injury.
By signing below, I also certify that my child(ren's) pictures may be used by Park Avenue Tennis Center LLC and NETA for promotional purposes on Park Avenue Tennis Center LLC and NETA website and in printed promotional items.
Payment due in full at registration. 24hr cancelation notice for daily drop-in.
